Adult Intellectual and Developmental Disabilities Support Services |
Adult Intellectual Disabilities Support Coordination Services ensures that service systems and community supports are responsive to the specific, multiple, and changing needs of individuals with intellectual and developmental disabilities. These services make certain that individuals and their families are properly linked to, and involved in, the appropriate services necessary to live successfully in their communities. ID Support Coordinators assist individuals in gaining access to appropriate housing, jobs, residential and day support services, social service benefits and entitlements, social and education resources, and psychiatric and medical services essential to meeting basic needs and achieving their goals. ID Support Coordinators provide comprehensive assessments to determine individual strengths, needs, and personal goals. They work in partnership with individuals and their families to develop an Individualized Service Plan that incorporates strategies to support and assist individuals in achieving their goals.
