This mobile multidisciplinary team provides intensive clinical treatment, rehabilitation, and support services to individuals experiencing severe and persistent mental illness. This team provides psychiatry, case management, counseling, nursing, vocational, and substance addictions services in the individual’s home or other community settings. These services are available 7 days a week, 24 hours a day, and 365 days a year on a long-term basis. The emphasis is to provide intensive wrap around services that allow the individuals to remain stable in the community and avoid psychiatric hospitalization. (Currently services are limited to individuals residing in Wythe and Smyth Counties.)
